Acute hospital
It is a hospital where they are hospitalized immediately after the onset or injury.
Occupational therapists provide rehabilitation for patients with cerebrovascular disease, spinal cord injury and heart disease, fractures and so on.
In Japan, Occupational therapy payments for medical services differ between patients with cerebrovascular disease and those with fractures. In addition, the price of rehabilitation is determined every 20 minutes. Rehabilitation is provided for up to 120 minutes per day, but it may be shorter depending on the number of PTs, OTs, and STs belonging to the hospital. Payment for medical services per 20 minutes varies depending on the size of the hospital and the number of therapists. All occupational therapy is provided by individual training.
Occupational therapy also provides rehabilitation for patients with mental and developmental disabilities, but at different prices.
Recovery Phase Hospital (Kaifukuki Rehabilitation Hospital)
This hospital is admitted to patients with cerebrovascular accidents, spinal cord injuries and femoral fractures. If they are severely ill, they can stay in this hospital for up to 180 days. Even patients who are judged not to be seriously ill can be hospitalized for 150 days. However, a fracture of the femur can only be hospitalized for 90 days. It provides 180 minutes of rehabilitation per day 365 days a year.
Prior to the coronavirus infection, families often came to the hospital to see them, but now they are restricted from entering the hospital. The time for family education provided by occupational therapists has become much shorter.
The occupational therapist visits the inpatient’s home with the patient and the patient’s family for a Home Evaluation and contacts the service provider to use after discharge.
Chronic hospital
The chronic hospital I have worked for was for the elderly living in the area.
Those hospitalized for undernutrition and dehydration were provided with cure and rehabilitation.
Many patient families wanted them to have a gastrostomy.
The spouses of some elderly patients, despite their age, came to visit every day.
They returned to their long-term care facilities and returned to their homes.
